Thyroid autoantibodies and speficially thyroid peroxidase antibodies (TPOab) are involved in the tissue destructive processes associated with autoimmune thyroid diseases. The appearance of TPOAb usually precedes the development of thyroid dysfunction.

Overview
Autoimmune thyroid disorders are thought to be caused by a combination of genetic and environmental factors. Risk factors include:
- a family history of autoimmune diseases
- being female
- pregnancy
- infections
- iodine intake
- certain medications
- stress
Elevated TPOab is seen in up to 90-95% of people with Hashimoto’s. Anti-TPO antibodies may act cytotoxic on thyroid gland cells in Hashimoto's.
Elevated TPOab is also seen in up to 80% of people with Grave's disease, but do not have an established role in the disease.
